REQUEST FOR PROPOSAL (RFP)
The Most Worshipful Prince Hall Grand Lodge of Free and Accepted Masons of Georgia, a philanthropic and non-profit fraternal organization headquartered in Atlanta, Georgia, is seeking a contractor to assist in the renovation of its kitchen facility space. The most responsive, responsible, lowest proposal will be selected.
Circumstance
The Most Worshipful Prince Hall Grand Lodge of Georgia headquarters at 330 Auburn Avenue was built in 1937 with additional floors added in 1941. The building architectural style is Renaissance Revival and the facility currently serves as a meeting place for the Prince Hall Masons. The kitchen space is located on the third floor of the location.
Tasks To Be Accomplished
• Kitchen Floor (Stone Tile) – Acid Wash of entire tile floor for grease removal – Sealant application for floor maintenance – Floor tile replacement during sink installation shift (4-5 tiles)
• Ceiling (Drop Ceiling) – Clean existing structure grid – Replace current ceiling tiles with vinyl ceiling tiles – Install two 2’ x 2’ florescent lights with in grid
• Plaster Walls – Install white washable white panels on South and West Wall. – Install rubber base boards along kitchen walls – Perform patching and paint the kitchen walls with high gloss paint. • Cabinets – Remove current cabinets on North Wall – Perform any associated patching and painting post removal (see “Plaster Walls” Section)
• Doors – Repair, sand and refinish North kitchen door – Sand, refinish and shave swinging East Kitchen Door (Door is solid wood)
• Sink – Installation of 3 division sink along West Wall – Installation of sink fixtures/faucets including a spray nozzle – Remove dead Water lines on West and North wall
How To Submit A Proposal
Interested contractors should submit a SEALED proposal, on or before 3:00 pm on April 15, 2008 to:

Faxed And Emailed Proposals Will Not Accepted.
Proposals Must Include:
1. A copy of the contractor’s current business license.
2. Documentation demonstrating the contractor’s ability to insure/bond the entire amount of the proposal, if selected.
3. A firm estimate of labor rates and costs to be charged, including an estimate of material expenses that would be incurred for the items listed in the “Tasks To Be Accomplished’ Section.
4. Photographs of past kitchen restorations from two locations.
* 5. Names, phone numbers and contact people for three references who have been the contractor’s clients during the last 18 months, whom we can call on as references.
Failure to submit any of the items requested in the proposal will
result in the proposal being deemed “non-responsive”.
